Patarillo! unfolds in a society that closely mirrors our own, except for one crucial difference: the source manga belongs to the genre of popular works aimed at adolescent Japanese girls, one that features a generous helping of gay men and beautiful teenage boys—the so-called bishounen. Most of the story is set in Marinera, a land of eternal spring located somewhere in the South Seas. The country's chief export is diamonds, and they form the backbone of the conflict that drives the anime. The gems come from one of the world's most abundant mines, owned by Marinera's boy-king Patarillo, who may be short on height but makes up for it in other proportions. The International Diamond Syndicate, a massive quasi-criminal secret society hell-bent on monopolizing the global diamond supply, covets that mine and will go to any lengths to seize it. In the early episodes, they dispatch a series of bishounen assassins to eliminate Patarillo, which forces him to rely on a bodyguard: Major Jack Bancoran, an MI6 agent nicknamed the "Bishounen-Killer." The nickname has nothing to do with him executing bishounen in cold blood—rather, it stems from the fact that no male under seventeen can resist his magnetic sexual appeal, a fact that includes Patarillo himself, much to Bancoran's perpetual chagrin. The narrative frequently shifts between Marinera and MI6 headquarters in London (which, oddly enough, appears to be a mere two-hour flight from the South Seas), as well as Bancoran's lavish condo in the city's outskirts. Notably, MI6's HQ resembles Fritz Lang's Metropolis, while Jack's apartment looks suspiciously like Randolph Hearst's estate. The story also ventures into the past, the future, and outer space. Patarillo evidently gets around.
