After taking part in a seemingly harmless fortune-telling game called "Persona," Naoya Toudou unexpectedly summons a distressed young girl. Her desperate pleas overwhelm him, causing him to wake in a surreal space where an enigmatic figure named Philemon resides. Philemon grants Naoya the ability to wield a "Persona"—a being that acts as an alternate self—before he returns to reality in the school infirmary. His friends also remember encountering this entity but dismiss it as they head to the hospital to visit their sick friend, Maki Sonomura. While visiting Sonomura during her surgery, she suddenly disappears, and the hospital's patients inexplicably turn hostile. Cornered, each member of the group summons their own Persona, successfully repelling the attackers, much to their amazement. Concerned for their missing friend, the group sets out to find Sonomura, delving deeper into the ominous situation they now face.
