High school brings many new experiences and classmates, but for Renge Ooyama, her introduction to one particular classmate is more peculiar than expected. Upon meeting Sumire Yotsuya, Renge realizes that Sumire is not the typical 16-year-old student she appears to be; instead, Sumire is a wooden puppet controlled by an elderly man dressed in black. Despite efforts from fellow students and staff to remove the man, he maintains his role and provides only perplexing answers through the puppet Sumire. Since the principal accepts Sumire as a genuine student, everyone else, including teachers and pupils, has no choice but to treat the puppet as a regular classmate. As Renge shares classes with Sumire, she feels uneasy when "she" insists on spending time together and becoming friends, fearing it could harm her reputation at school. However, when Renge faces difficulties, it is Sumire who steps in to help. Recognizing that Sumire—and the elderly man—are not entirely malevolent, Renge decides to give "her" a chance. Sumire♡16-sai!! chronicles Renge and her peers as they adapt to school life with this unusual addition and gradually uncover that, despite Sumire's wooden appearance, her heart is truly authentic.

Sumire♡16-sai!! was previously serialized in Weekly Shounen Magazine until November 8, 2006 (2006 #48). It was then moved to Magazine Special on December 20, 2006 (2007-01). A special chapter was published in Weekly Shounen Magazine on April 11, 2007 (2007 #19). The series was adapted into a 12-episode TV Drama which aired from April 13, 2008 to June 29, 2008.
