Six months after Sakuta Azusagawa first met the bunny girl Mai Sakurajima in a library, the pair are enjoying a happy relationship. However, Sakuta's life is thrown into chaos when Shouko Makinohara, his childhood crush who has since grown up, suddenly returns. Complicating matters further, Sakuta encounters a younger, middle school-aged version of Shouko in a hospital, where she is fighting a critical illness. Strangely, his old scars begin to ache whenever he is near her. As Shouko's peculiar situation seems linked to him, Sakuta must confront his conflicting feelings regardless of the cost. With a young girl's life hanging in the balance of his choices, what actions can he take?

In "Rascal Does Not Dream of a Sister Venturing Out," Kaede Azusagawa, the younger sister of Sakuta, is nearing the end of middle school and must make a major choice. Despite her past as a shut-in and severe anxiety in the classroom, which led her counselor to suggest an online high school, Kaede longs to attend Minegahara High School with her brother and enjoy a typical high school experience. She seeks help from Sakuta, his girlfriend Mai Sakurajima, and Mai's half-sister Nodoka Toyohama to prepare for the entrance exam, and they agree to support her. However, her high school dreams prove more difficult than expected as her lingering trauma resurfaces, forcing Kaede to confront and determine her own path forward.
