The story of Hantsuki centers on the developing romance between seventeen-year-old Yuuichi Ezaki and Rika Akiba. Both are patients in Yuuichi's hometown hospital due to their illnesses: Yuuichi suffers from hepatitis A, while Rika has a weak heart valve. As they spend time together, the two teenagers fall in love. The story is set in Ise, Mie Prefecture, as indicated by the "Welcome to Ise" sign that appears after the opening sequence of the anime's first episode.
