Senpai wa Otokonoko is based on the web manga Otokonoko ga Kouhai ni Kokuhaku sareru Hanashi (おとこのこが後輩に告白される話), released on the author's Twitter and pixiv accounts from July 3, 2019, to July 24, 2019. The series was published digitally in English as Senpai Is an Otokonoko by Webtoon Entertainment from March 9, 2023, to February 8, 2024. Webtoon Entertainment also released the series in French from March 28, 2022, to February 5, 2024; and in German since May 28, 2022. The volumes have been published in English as Senpai Is an Otokonoko: My Crossdressing Classmate by Kodansha USA since June 17, 2025.
